There is an exciting opportunity at Caerphilly County Borough Council for a Qualified Social Worker to join our well-established Older People Social Work team, covering the North of Caerphilly County borough.The North Older People Team is based at the Rhymney Integrated Health and Social Care Centre alongside district nurses, two GP practices and other health and social work teams. We are an enthusiastic and dedicated team who strive to support older adults and their carers, who are in need care and support.The teams deliver services to older adults, who have mental health or physical health needs. A high proportion of our work involves supporting those who have a dementia diagnosis.The post will involve working with older adults to help them find appropriate solutions at times of difficulty, as well as in the long term. It will also entail safeguarding individuals from abuse and neglect.The post holder will undertake assessments, reviews and contribute to the duty rota within the team.
